php search form and results form modification

hai any body can help me for my institute search form customization in the search form it is using id number and course ( two validities) i want only by id and 2nd one output form ( result form ) in that all the data is ok but i wnt grade based on individual marks and total marks and final pass or fail .below i am attaching the two files.
